Directions on completing the forms

Form 1 - Titled 'Workforce Investment Act (WIA) Customer Pre-Application Information

Pages 1 and 2 are pretty much self explanatory, just fill in the requested information.

On Page 3, near the bottom, is the section titled 'Household and Income Information'. For the 'Total Number in Household' field, include all persons living in the household, even if they are not related to you.

For the space labeled 'Monthly Gross Household Income', total all income from the individuals you included in 'Total Number in Household'. Remember, this asks for GROSS income, which is the income before anything (taxes, medical, retirement, social security, Medicare, etc.) is deducted.

Page 4 is the breakdown of the information you provided on Page 3. List all of the individuals that you included in your 'Total Number in Household' count on Page 3. On the bottom half of the page, detail the household income.

Form 2 - Titled 'Workforce Solutions of Central Texas Centers Assessment of Practical Needs'.

On these two pages answer all of the Yes/No questions. DO NOT answer the questions on the second page under the FINANCIAL heading. A Workforce Specialist will answer those and sign at the bottom of the page after you have turned in the packet.

Form 3 - Titled 'Workforce Solutions of Central Texas Centers WIA General Assessment'

This information will help us to have a better understanding of you and the events that shape your life. Answer the questions as completely as possible.

The questions are self-explanatory. DO NOT write below the section titled 'Program Use' on Page 5.

Form 4 - Titled 'WIA Financial Assessment Form'

(1 page)

On this form, list your monthly income (Note: On wages it asks for the 'take home' pay after expenses have been deducted. This is different than previously where you were asked to list your GROSS income.)

Form 5 - Titled 'Workforce Solutions of Central Texas Centers Orientation to Complaint Procedure'

(1 page)

This form shows you where to turn if you feel that you have been treated wrongly or unfairly in your dealings with the Workforce Solutions of Central Texas Centers.

Form 6 - Titled 'Core Services'

(2 pages)

DO NOT fill this form out. Take these two pages to any Workforce Solutions of Central Texas Centers and use their services to perform the tasks listed on the 'Core Services Attachment' sheet. After you have done that, a Workforce Technician will complete the form and sign it.


After all of these forms have been completed, turn them in at the nearest Workforce Solutions of Central Texas Center.
